## Real therapy, differently located

Telehealth is not therapy-lite. The session structure, the methods
([CBT](/therapy-services/cognitive-behavioral-therapy) worksheets and all), the confidentiality
and the clinical standards are identical to the office; the change is the room, and sometimes the
change of room helps. Plenty of people say hard things sooner from their own sofa. Our providers
are licensed in both Maryland, Washington, D.C. and Ohio, so care follows you across that particular
border without missing a week.

## How to set up a session that actually works

Small things make a disproportionate difference to a video hour.

- **Somewhere private, for the whole hour.** This is the one genuine constraint. Patients get
  inventive (a parked car, an office after hours, a walk with headphones) and any of those beats
  a kitchen where someone might wander through.
- **Headphones.** They improve your privacy and the audio at once, and they make it far easier to
  stay in the conversation.
- **Camera at eye level**, with light in front of you rather than behind. A window at your back
  turns you into a silhouette, and faces carry a lot of what a therapist is reading.
- **Notifications off**, other tabs closed. The session competes with a device designed to
  interrupt.
- **Join a couple of minutes early**, so a microphone permission dialog does not consume the first
  five minutes of your hour.
- **Have a fallback agreed**, usually a phone call, so a dropped connection is an inconvenience
  rather than a lost session.

Two things worth saying out loud to your provider: if you are somewhere you cannot speak freely,
and if the screen is making something harder. Both are workable, and neither is obvious from the
other side of a camera.

## What it unlocks, practically

For our patients the honest advantage is arithmetic: sessions that happen. The Eastern Shore
patient who will not drive hours each way; the parent whose childcare window is exactly one hour;
the ADHD patient for whom the barrier was always the leaving, not the talking; the anxious
patient for whom the waiting room was the worst part. Across a course of therapy, attendance is
the strongest predictor there is, and video is an attendance technology.

## Frequently asked questions

### Is online therapy actually private and secure?
Sessions run over secure, healthcare-grade video, not consumer apps, and the same confidentiality rules as office visits apply. Your half of privacy is a room where you can speak freely; patients get inventive here, and parked cars, offices after hours and headphones on a walk all have precedents.
### What do I need for it to work?
A phone, tablet or computer with a camera; a reasonable connection; and somewhere private for the hour. That is genuinely all. If a session drops mid-call, your provider reconnects or phones you: technology hiccups are a normal part of telehealth, not a crisis.
### Are there things online therapy can't do?
A few. Some situations, assessments and medication types need an in-person visit, and severe crises need faster resources than any scheduled session (call or text 988 in a crisis). Your provider tells you plainly when in-person matters for you; for everything else, our offices remain available whenever you want a room instead of a screen.
### Does online therapy work as well as meeting in person?
For most common conditions the research is reassuring: video-delivered therapy performs comparably to in-person work for anxiety, depression and several other presentations, particularly with structured methods such as CBT that translate cleanly to a screen. The bigger practical factor is attendance. A session that happens by video beats an in-person session that the commute cancelled.
### Can I have online therapy if I am outside Maryland or D.C. that day?
Ask before you travel, because the rule is about where you are sitting, not where we are. Clinicians are licensed by state, and telehealth is generally governed by the patient's location at the time of the session. Our providers cover Maryland, Washington, D.C. and Ohio. If you will be elsewhere (a holiday, a move, a semester away) tell your provider in advance so the schedule can be planned rather than interrupted.
